WYANDOTTE HEALTH FOUNDATION
2010-2011 GRANT GUIDELINES AND APPLICATION*
Overview and History
The Wyandotte Health Foundation (WHF) was created by the sale of Bethany Medical Center in October 1997. WHF is an independent 501(c)(3) organization not affiliated with any hospital. It continues, however, to maintain the commitment of the former Bethany hospital to serve the citizens of Wyandotte County, Kansas. WHF is governed by an independent Board of Directors that sits as a committee-of-the-whole to evaluate and award grants.
Grant Making Priorities
WHF's mission is to promote and improve the health of Wyandotte County citizens, particularly the indigent, through grants and collaborative efforts. Therefore, WHF seeks to achieve this mission by focusing its grant making into two major categories:
- Prevention, Intervention & Education
Programs/projects that directly and positively change the health habits, behaviors and/or lifestyles of the indigent, uninsured and underinsured of Wyandotte County through prevention, intervention and education strategies.
- Primary Health Care
Programs/projects that deliver affordable and accessible primary health care directly to the indigent, uninsured and underinsured of Wyandotte County. Primary care is performed and/or managed by a personal physician often collaborating with other health professionals, and utilizing consultation or referral as appropriate. Primary care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses in a variety of ambulatory health care settings.
For 2010 – 2011, primary focus will be given to Primary Health Care, with a secondary focus on Prevention, Intervention & Education projects. Highest consideration for funding will be given to programs or providers of projects that:
- Are located in Wyandotte County and directly serve Wyandotte County residents
- Increase providers' ability to support and effectively deliver primary health care services to the indigent, uninsured and underinsured
- Are collaborative efforts among health care providers
- Demonstrate an effort to minimize duplication
- Leverage additional funding, both private and public
- Do not replace or supplant existing funding
- Demonstrate cultural sensitivity (both interpretation/translation services and cultural awareness
- Have clearly established measurable outcomes and the ability to monitor and report them
